Compulsory membership declaration
I hereby apply for membership of AlphaTotas.
I agree that if this application for membership
is accepted I will be bound by the provisions
of the policy and the rules, regulations,
by-laws and procedural policies of AlphaTotas
as laid down from time to time, including
any disciplinary procedures.
I agree to conform to the Code of Ethics
of AlphaTotas.
I confirm that the information provided by
me in this form is in all respects correct
and complete to the best of my knowledge
and belief and I consent to it being used
by AlphaTotas in considering my membership
application or the imposition of disciplinary
procedures.
I acknowledge that AlphaTotas reserves the
right to refuse my application for membership
without assigning any reason.
I agree that AlphaTotas may use my personal
information in the provision, administration,
improvement or promotion of products and
services relevant to AlphaTotas’s membership
activities.
I agree to abide by AlphaTotas’s rules
with respect to payment of subscriptions
and fees. I understand that AlphaTotas is
a company limited by guarantee and that my
guarantee is limited to £2. I understand
that under the policy, Certificates of Membership
remain the property of AlphaTotas and must
be destroyed on cessation of Membership.
